Your Appointment Is Booked
Now that you’ve made your appointment with a broker or agent, you can create an account with Your Health Idaho, if you haven’t already done so already. Create your account by going to the top of this page and clicking “Login” (as shown in the image on the right). Then select “Create an Account” from the drop-down menu.
You’ll enter some basic information and set a password.
Your Application
Once you have an account, you’re ready to start your application, or complete it, before meeting with your agent or broker. Click “Login” on the menu above, then select “Login” (shown right).
If you’ve finished your application and would like to change the plan you previously selected, you can do that now.
Here’s a list of the documents and information you may need to reference during the application process:
- Social Security numbers
- Proof of citizenship or immigration status
- Proof of tribal status
- Tax returns from previous years
- Employer / income information (for example, W2s or paystubs)
- Current insurance policy information
Please complete as much of the application as you can before your meeting. If you have questions while you’re filling it out, you can go to our Frequently Asked Questions page. You can also make note of the questions and share them with your agent or broker when you get together.
If you complete the application and see a red box on your dashboard that says, “This eligibility is conditional,” you may need to submit documentation. Click See More Details to view what documents you need to provide. You can upload them yourself in advance or bring them to your meeting.
Allowing Access to Your Account
To grant your agent access to your account so they can provide more direct assistance, follow these steps while you’re in the application:
- In the top menu bar, select Get Assistance > Find Local Assistance
- Select “Find a YHI Certified Agent or Broker Near You”
- Under “Search by name” enter your agent’s name
- Click their name, then Select Agent on their profile
- Complete the steps to designate them as your agent
Preparing for Your Meeting
Things to bring to your meeting:
- A list of all current doctors and medications
- Information about your annual income, including tax returns or paystubs
- Any questions you may have
